Record Ex+/ sleeve Ex) and Still Life (CAS 1116, UK small Mad Hatter labels. Record Ex+/ sleeve Ex+). Both sleeves have a John Peel archive numbered sticker on the top left corner of the cover, hand-written by Peel himself.
